I agree to allow my child to participate in the IEA Eventing Clinic with Lillian Heard Wood on May 3-4, 2025 and understand all the risks involved.

In consideration of the risk of injury while participating in the IEA Eventing Clinic with Lillian Heard Wood (the "Activity"), and as consideration for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my child's participation in the Activity and do hereby release and forever discharge the Indiana Eventing Association, their affiliate, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any physical or psychological injury, including but not limited to illness, paralysis, death, damages, economical or emotional loss that I may suffer as a direct result of my child's participation in the aforementioned Activity, including traveling to and from an event related to this Activity.

Johnson County Horse Park Waiver

Johnson County Parks has their own waiver for all equine participants to sign.

JOHNSON COUNTY PARKS’ EQUINE RIDING/SCHOOLING WAIVER(Required)

ATHLETIC WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to participate at a Johnson County Parks and Recreation Department facility and/or related events, the undersigned:
1. Agree that prior to participating, the participants, or the parent(s) or legal guardians of the minor participants, each will inspect the facilities and equipment to be used, and if they believe anything to be unsafe, they will immediately advise their coach or supervisor of such condition(s) and refuse to participate.
2. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability, or death.
4. Release, waive, discharge and covenant not to sue Johnson County Parks and Recreation Department, Johnson County Commissioners, and Johnson County Soil and Water Conservation District, and their affiliated clubs, their respective administrators, directors, agents, coaches and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as “releasees”, from any and all liability to each of the undersigned, his or her next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.

THE UNDERSIGNED HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTANDING THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY.
